New Client Sign Sheet: If you fill out this new client information sheet and bring it with you to the appointment it will help us process your new file for efficiently.
If you have the ability to fax (212-685-2857) or e-mail (drdan@vetnyc.com) the completed new client sheet it to Murray Hill Pet Hospital before your appointment that would be preferred.
What to do for your first appointment
1. It is important to make sure you have your pet's prior health records faxed (212-684-7633) or e-mailed (drdan@vetnyc.com) to us before your visit. If this cannot be performed then try to bring the records with you to the appointment.
2. If your pet is on any medication than bring it with you to the visit.
3. It is never wrong to bring a fecal sample to the visit.
